At Gracefully Meek Home Care LLC. in Chiefland, FL the first thing we will do is to contract with your family and send a nurse to your home.
We will perform a skilled nursing assessment. During the session, the nurse will identify the needs, preferences, and abilities of the patient. The assessment includes observation and interview with the patient.
The nurse will consider the symptoms and signs of the condition; the patient’s verbal and nonverbal communication; the patient’s medical and social history; medication and chart reviews, and any other information available from the family, doctors, lab results, diagnostic test results, social workers, nutritionist, therapist, etc. Next, we will complete a physical assessment.

- Caregiver Section – The Caregiver questionnaire section helps assess the caregiver’s experience, needs, and challenges, as well as their perspective on the caregiving relationship, and helps improve care coordination.
Care Plan
- Goals:
Specific, measurable, achievable, relevant, and time-bound (SMART) goals for the patient’s care, focusing on improving health, reducing pain, and/or increasing independence.
- Interventions:
Specific actions and services that will be provided to help the patient achieve their goals, such as medication management, wound care, physical therapy, or personal care assistance.
- Caregiver Team:
Identifies all caregivers involved in the patient’s care, including their roles, responsibilities, and contact information.
- Schedule and Routine:
A daily, weekly, and monthly care schedule that aligns with medical needs and patient preferences.
- Regular Review and Adjustment:
The care plan is reviewed regularly to assess progress, adjust interventions, and address any changes in the patient’s condition.
The assessment is extremely thorough and important because it provides the scientific basis for a complete nursing care plan to be implemented. This will help us focus on the uniqueness, desires, needs, and strengths of each patient and his/her family. All information disclosed, guided by our nursing expertise, will allow registered nurses to become advocates for our patients as we manage the care they receive.
Our skilled nurses will visit clients receiving unskilled care at home at least every 60 days or as contracted. This will allow us to monitor a change in the patient’s condition and connect with the physician for a comprehensive report and update. We aim to be informed and updated enough after each assessment to advocate for our patients.
